Professional case managers advocate for, coordinate, and monitor services to improve the quality of life for individuals with special needs. Case management services tend to be more intensive and multifaceted than consultation services, with longer term follow up.

How does it work? The case manager meets with you and develops an individualized plan that targets the most crucial issues impacting quality of life and long term independence. The plan can address multiple issues or focus on one specific issue. In many cases, case managers also work one-on-one with clients to develop their communication skills, identify more productive coping strategies, expand social contacts, improve employability, develop improved problem solving skills, etc.

This is a sliding-scale fee-based service.
